Licensed Practical Nurse (LPN) Home Health

Billet Health | Per Diem or Full-Time Caseload

Billet Health is seeking compassionate and skilled Licensed Practical Nurses (LPNs) to join our Home Health team serving geriatric patients throughout a 9-week episode of care.

As part of our clinical team, you will work closely with RN Case Managers and the Director of Nursing to deliver high-quality, patient-centered care in the home setting.

Position Overview

This role provides in-home skilled nursing care with flexible scheduling and visit-based assignments. Our staffing team coordinates patient visits based on clinical needs, and LPNs have the flexibility to build their own caseload.

Typical visits range from 4560 minutes.

Responsibilities

• Provide skilled nursing care in the home setting
• Support RN Case Managers in implementing care plans
• Monitor patient condition and report changes
• Assist with patient education and ongoing care coordination
• Document care accurately and timely
• Maintain patient safety and quality standards

Qualifications

• Active LPN/LVN license in the state of practice
• Valid drivers license
• Valid auto insurance
• CPR or BLS certification
• Current TB test (skin test within 1 year or chest X-ray within 5 years)

Additional Information

Care is provided in patients homes or residential facilities. Compensation includes travel within a 20-mile radius, with additional pay opportunities for extended travel.

New graduates and experienced nurses are welcome. Training is provided for Home Health onboarding.

Compensation

• $45.00/hour starting pay (based on experience)
• Per diem or full-time availability
