RN Case ManagerSchedule7:30 AM - 4:00 PM, Monday through Friday with every third weekend and holiday coverage required.Job DescriptionWe are seeking a skilled and experienced RN Case Manager to join our Care Management team. The ideal candidate will have a strong background in discharge planning and care coordination, and will be responsible for managing the care of patients, ensuring smooth transitions and continuity of care.ResponsibilitiesConduct comprehensive assessments and develop care plans for patients.Collaborate with healthcare teams to coordinate patient care and services.Facilitate discharge planning processes to ensure safe transitions.Educate patients and families on conditions, care plans, and resources available.Ensure compliance with regulatory requirements and quality standards.Required Experience / Certifications / LicensureMinimum of 5 years of experience in hospital case management.Minimum of 5 years acute care nursing experience in a hospital setting.Current RN license in the state.Experience with EMR systems, specifically EPIC and Wellsky.LocationPortland, MaineWhy ARMStaffing?At ARMStaffing, we take care of our employees!
