Our client is seeking a compassionate and skilled Home Health Physical Therapist (PT) to provide care to patients in Roswell, NM. This is a great opportunity for a therapist who values flexibility, autonomy, and meaningful one-on-one patient care in a home setting.

Key Responsibilities:

• Evaluate patients and develop individualized treatment plans in a home health setting
• Deliver hands-on therapy to improve mobility, strength, and overall function
• Educate patients and caregivers on exercises and care plans
• Track progress and adjust treatments as needed
• Complete accurate and timely documentation

Qualifications:

• Active Physical Therapist license in New Mexico
• Current CPR certification
• Valid driver's license and reliable transportation
• Strong communication and organizational skills
• Home health experience preferred
