Physical Therapist (PT) – Full Time | Skilled Nursing Facility

Location: Mahoning Post Acute Nursing and Rehab

Job Type: Full Time

We are seeking a compassionate and motivated Physical Therapist to join our Skilled Nursing Facility team. This role is ideal for a clinician who thrives in a collaborative, patient-centered environment and is passionate about helping older adults regain function, maintain mobility, and improve their quality of life.

Key Responsibilities:

• Perform comprehensive evaluations for residents with orthopedic, neurological, cardiopulmonary, and medically complex conditions

• Develop individualized treatment plans tailored to resident goals, functional needs, and SNF regulatory guidelines

• Provide therapeutic interventions including strengthening, balance training, gait training, transfer training, pain management, and functional mobility retraining

• Collaborate closely with nursing, occupational therapy, speech therapy, and social services to support interdisciplinary care

• Document evaluations, daily notes, progress reports, and discharge summaries in accordance with Medicare, state, and facility requirements

• Educate residents and caregivers on safe mobility, fall prevention, and home exercise programs

• Maintain a safe and supportive therapy environment

Qualifications

• Active state Physical Therapist license

• Excellent communication, documentation, and clinical reasoning skills

• Experience in geriatrics or skilled nursing strongly preferred, but new graduates with a passion for older adults are welcome
