Job Description
Title: Home Health RN - Nursing

Schedule:
Shift: 8-hour days only
Weekend requirement: No
On-call: No

Assignment Details:
Typical visits per day: 5 (may visit up to 6)
Productivity expectation: ~30 points standard

Requirements:
- Oregon RN license required
- BCLS required
- Minimum: 1 year skilled home health experience
- OASIS experience required
- Clinical skills: Admission/Start of Care, Routine visits, Medication administration, Wound care (including V.A.C.), G-tube care, Catheter and PICC care

Roles & Responsibilities:
- Provide skilled nursing care in a home health setting
- Conduct patient assessments and develop care plans
- Administer medications and treatments as prescribed
- Educate patients and families about managing health conditions
- Coordinate care with interdisciplinary teams
- Monitor and document patient progress

Compliance / Notes:
- Dress code: Business casual preferred; scrubs allowed (no jeans)
- Facility may require COVID-19 vaccination, including primary series and booster
- Orientation: 40 hours required
