Physical Therapist

Position Summary The Physical Therapist is responsible for evaluating, assessing, and treating individuals to improve mobility, physical function, and overall quality of life. The role involves developing individualized rehabilitation plans, monitoring patient progress, collaborating with interdisciplinary teams, and maintaining accurate clinical documentation. Key Responsibilities Perform comprehensive physical therapy evaluations using standardized tests and clinical assessments. Determine rehabilitation needs and develop individualized treatment plans. Provide or oversee ongoing patient assessments and modify treatment plans based on progress. Document evaluations, treatment plans, progress notes, and assessment results accurately. Communicate findings and recommendations to interdisciplinary healthcare teams. Participate in developing and presenting individualized treatment goals. Conduct periodic reassessments and adjust therapy interventions as needed. Participate in continuing education, in-service training, research projects, and program development. Required Qualifications Active Ohio Physical Therapy License Healthcare Provider CPR Certification Relevant experience as a licensed Physical Therapist Ability to pass required background/security screening TB Test and Drug Screen initiated within 48 hours of the start date if selected Employment Type: Full Time Salary: $40 - $46 Hourly Bonus/Commission: No
