Hospice Nurse Practitioner

The Hospice Nurse Practitioner works collaboratively with the Medical Director, hospice physicians, and interdisciplinary team to provide high-quality hospice services to patients and families across multiple care settings. Responsibilities include clinical assessment, symptom management, care planning, patient and family education, and coordination of services. This role includes coverage in the Inpatient Unit (IPU) and home-based care, including required face-to-face visits.

Primary & Essential Duties

• Provide direct clinical care including assessments, diagnosis, treatment planning, and follow-up care.
• Complete hospice face-to-face visits and eligibility evaluations.
• Provide care in the IPU and in patient homes.
• Conduct routine IPU rounding including symptom management and documentation.
• Prescribe medications and order treatments within scope of practice.
• Collaborate with physicians and interdisciplinary teams for coordinated care.
• Educate patients, families, and staff on hospice care.
• Maintain accurate and timely clinical documentation.
• Ensure regulatory compliance and adherence to agency policies.

Qualifications

• Current Wisconsin Nurse Practitioner license and masters degree in nursing.
• Strong clinical judgment, communication, and interpersonal skills with the ability to work effectively with diverse populations
• Demonstrates professionalism, tact, and sound decision-making in all interactions
• Clinical expertise in medications, sterile techniques, and safety practices (PPE)
• Effective leader and educator with strong collaboration and independent work capabilities
• Proficient in computer use and clinical documentation
• Self-directed, adaptable, and able to manage stress and emergency situations
• Meets driving requirements, including valid license, reliable transportation, and insurance

Schedule

• Full-time salaried position working Monday Friday from approximately 8am-4:30pm.
• On-call coverage is required of at least one weekday per week from 4:30 PM to 7:00 AM and one weekend per month from 4:30 PM Friday to 7:00 AM Monday.
• SSRCH territory includes Manitowoc, Sheboygan, & Ozaukee County.

Working Conditions

• Physical Requirements: Frequent ambulation; constant bending, lifting, twisting, pushing and pulling; reposition patients, lift minimum of 30 pounds unassisted; bilateral manual dexterity; climb up and down stairs; Job may include sitting, standing, bending, stretching, stooping, walking, lifting 20 50lbs and repetitive hand motion associated with keyboarding; drive to and from patient homes; able to sit for extended periods of time; manual dexterity to operate computer and other office equipment.
• Mental Requirements: Read, speak, write, and understand English; cognitive processes to interpret patient condition and apply appropriate clinical processes in complex situations; ability to add, subtract, multiply, divide and work with fractions; ability to drive and locate patient homes; must function independently without supervision while working in collaboration with the IDT (Interdisciplinary Team).
• Working/Environmental Conditions: Fast-paced changing environment, with multiple directives from several disciplines; may at times require work in patient homes with no control over environmental conditions. Potentially unsafe and unsanitary home environments. Potential travel in inclement weather. Work assigned throughout the agencies clinical program setting.
• Machines, Equipment, Tools Used: Including but not limited to; automobile; telephone; smart phone; computer, laptop, or tablet; fax machine.
• Exposures: Anticipated exposure to blood, other body fluids or other potentially infectious materials. Exposure to communicable diseases or infections. Anticipated exposure to hazardous equipment, assorted chemicals, gases/vapors, and other pollutants.
• Personal Protective Equipment Required: Assorted gloves; assorted masks, protective eyewear; impervious gowns; impervious aprons; needleless systems as appropriate.
