Home Health Field LVN/LPNThe Home Health Field LVN/LPN plays a critical role in delivering high-quality nursing care to patients within their homes, ensuring comfort, safety, and effective management of health conditions. This position requires the ability to assess patient health status, administer medications, and provide wound care while collaborating closely with physicians, therapists, and family members. The LVN/LPN will be responsible for developing and implementing individualized care plans that promote patient independence and improve overall well-being. This role demands strong clinical judgment and compassionate communication to educate patients and caregivers on treatment protocols and health maintenance.
