Licensed Practical Nurse (LPN)

A skilled nursing and rehabilitation facility in the Clifton Springs area is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our care team. The ideal candidate will be reliable, professional, and committed to providing excellent care to residents in a supportive long-term care environment. Duties Collect and record resident health information, including vital signs and medical details Administer medications and injections as prescribed by providers Provide wound care and assist with dressing changes and other treatments Supervise Certified Nursing Assistants (CNAs) as needed Support residents with personal care and daily living activities Monitor and document residents’ nutritional and fluid intake/output Collaborate with the nursing team to maintain the highest standards of care Requirements Valid New York State LPN license in good standing Strong teamwork and communication skills Compassionate approach to resident care Prior long-term care experience preferred About the Facility Our long-term care and rehabilitation facility is dedicated to helping residents achieve the highest quality of life in a warm, supportive environment.
