JOB SUMMARY
The Care Manager is responsible for overseeing the client care experience from assessment through ongoing service. This position conducts client assessments, develops and maintains individualized care plans, completes required client and caregiver supervisory visits, and provides ongoing support and education to caregivers.
The Care Manager serves as a primary connection between clients, families, caregivers, and the Right at Home office team, proactively identifying changes in needs, addressing concerns, and helping ensure services are delivered safely, consistently, and in accordance with each client's care plan and Right at Home standards.
ESSENTIAL FUNCTIONS Client Care & Care PlanningConduct new client assessments to identify care needs, preferences, routines, risks, and expectations and support a successful start of care.
Develop, maintain, and update individualized care plans to accurately reflect current client needs and services.
Conduct quarterly client visits to evaluate satisfaction, review services, identify changes in condition or needs, and strengthen client and family relationships.
Identify changes in client condition, needs, or circumstances and coordinate appropriate updates to care plans and services.
Assist in resolving client or family concerns and support service recovery and client retention when issues arise.
Conduct supervisory visits to observe care delivery, ensure adherence to the client's care plan, reinforce expectations, and provide coaching as needed.
Train and educate caregivers on client-specific needs, routines, care plans, procedures, and changes in care.
Provide ongoing support and direction to caregivers and communicate performance, safety, or care concerns to appropriate office team members.
Help identify and address caregiver-client compatibility or care delivery concerns that may affect the quality or continuity of services.
Maintain regular communication with clients and families regarding care needs, changes in condition, caregiver concerns, and services.
Monitor the Family Room and other designated communication channels and ensure messages, questions, and concerns receive timely follow-up.
Collaborate with scheduling, recruitment, operations, and leadership to support continuity of care and a positive client and caregiver experience.
Respond promptly and professionally to concerns, incidents, complaints, or changes in condition and escalate issues to leadership when appropriate.
Assist prospective clients and families in understanding Right at Home's services and approach to care when needed.
Maintain accurate and timely documentation of assessments, client visits, supervisory visits, care plan updates, changes in condition, incidents, and significant communications.
Protect client confidentiality, maintain appropriate professional boundaries, and follow HIPAA, applicable state regulations, company policies, and Right at Home standards.
Participate in emergency and care-management decision-making during designated office or on-call hours as assigned.
Build positive, professional relationships with clients, families, caregivers, referral partners, and community members.
Represent Right at Home professionally and demonstrate compassion, reliability, accountability, responsiveness, and a commitment to excellent service.
Perform other duties as assigned to support client care, caregiver success, and agency operations.
Minimum of two (2) years of experience in home care, senior care, healthcare, or a related field.
LPN preferred.
Previous experience with client assessments, care planning, caregiver supervision, or case management preferred.
Working knowledge of HIPAA and applicable home care regulations.
Strong interpersonal, customer service, and relationship-building skills.
Excellent verbal and written communication skills.
Strong technology proficiency, including the ability to confidently navigate multiple software applications, email, web-based platforms, and digital communication tools; quickly learn new systems; and accurately complete documentation and other job responsibilities electronically.
Ability to assess situations, identify concerns, make sound recommendations, and escalate issues appropriately.
Strong organizational and time-management skills with the ability to prioritize multiple responsibilities and follow through consistently.
Ability to work effectively with clients, families, caregivers, and team members from diverse backgrounds.
Ability to coach, educate, and provide constructive feedback to caregivers.
Sound judgment and the ability to remain professional and responsive in challenging or time-sensitive situations.
Strong documentation and record-management skills with attention to accuracy and confidentiality.
Ability to understand, follow, and consistently apply agency policies, procedures, care standards, and regulatory requirements.
Work is performed in both the local office and client care settings, including private residences and other community-based environments.
Regular local travel is required for assessments, client visits, supervisory visits, and other business needs.
Must possess a valid driver's license, reliable transportation, and automobile insurance as required by law.
Must be available during designated hours of operation and participate in emergency or after-hour care-management coverage as assigned.
Ability to maintain flexibility in scheduling based on client, caregiver, and agency needs.
