Physical Therapist (PT) – Home Health | Hilo, HI | Part-Time & PRN | $65–$85 Per Visit

Stern at Home Therapy is expanding to Hilo and the Big Island and is hiring Part-Time and PRN Physical Therapists to help us grow our newest Hawaii service area.

We are already established on Oahu and are now expanding our home-based therapy services to the Hilo community.

This is an opportunity to join a growing market early, build a consistent local caseload, and help establish Stern at Home Therapy's presence on the Big Island.

What You'll Do:

As a Physical Therapist with Stern at Home Therapy, you will provide one-on-one care to a primarily geriatric patient population in their homes throughout Hilo and surrounding communities .

You'll have the clinical autonomy to evaluate patients, develop individualized plans of care, and manage your caseload around your availability. Our administrative team supports you with referrals, scheduling, authorizations, and patient coordination.

Responsibilities include:

• Perform physical therapy evaluations and develop individualized plans of care.
• Provide one-on-one physical therapy treatments in patients' homes.
• Help patients improve strength, balance, mobility, endurance, and functional independence.
• Provide gait training, transfer training, fall-prevention strategies, and home exercise programs.
• Recommend assistive devices, durable medical equipment, and home safety modifications when appropriate.
• Complete accurate and timely clinical documentation.
• Communicate with physicians, caregivers, families, and the interdisciplinary care team.
• Work with our administrative team as we continue developing the Hilo service area.

Compensation:

Part-Time W-2 – Minimum 10 Patients.

• $65 per follow-up visit.
• $75 per 4-unit evaluation.

Part-Time 1099 – Minimum 10 Patients.

• $72 per follow-up visit.
• $85 per evaluation.

PRN – Minimum 5 Patients.

• $72 per follow-up visit.
• $85 per evaluation.

Part-time and PRN compensation is paid at a fixed per-visit rate and does not use a tiered volume model.

Part-Time W-2 Benefits:

Eligible Part-Time W-2 employees may receive:

Qualifications:

• Active Physical Therapist license in Hawaii required.
• Graduate of an accredited Physical Therapy program.
• Reliable transportation required.
• Strong clinical, communication, documentation, and time-management skills.
• Ability to work independently in a home-based setting.
• Home health or geriatric experience is preferred but not required.
