Field-Based RN Case Manager - Wound Care
Adaptive Home Health
Adaptive Home Health is seeking an RN Case Manager to deliver high-acuity home health care with a focus on wound care and infusion therapy. You will own patient assessments, coordinate with physicians and care teams, and ensure safe, effective plans of care in the field. You will drive care coordination, execute timely follow-ups, and educate patients and families while navigating the scheduling and on-call rotation. Texas licensure and CPR are required. #J-18808-Ljbffr Adaptive Home Health
