You’ll collaborate with physicians, interdisciplinary teams, and community health professionals to ensure seamless, compassionate care. Key Responsibilities Assess and evaluate patient status Develop and revise individualized care plans Provide palliative nursing care and education Act as Case Manager when assigned Maintain accurate clinical documentation Communicate with physicians and community partners Qualifications Homecare Homebase/Point Care EMR experience is an added bonus.
