Full-Time Certified Occupational Therapy Assistant (COTA) – Skilled Nursing & Rehabilitation

Uniontown Nursing & Rehabilitation is seeking a compassionate and motivated Full-Time Certified Occupational Therapy Assistant (COTA) to join our in-house rehabilitation team in Uniontown, Pennsylvania . This is an excellent opportunity to work alongside experienced therapy professionals dedicated to helping residents maximize independence and improve their quality of life.

• Provide skilled occupational therapy treatments under the direction and supervision of a licensed Occupational Therapist.

• Implement individualized treatment plans for short-term rehabilitation and long-term care residents.

• Assist residents in improving activities of daily living (ADLs), upper extremity function, mobility, coordination, and overall independence.

• Monitor resident progress and communicate changes to the supervising Occupational Therapist.

• Educate residents and caregivers on therapeutic exercises, adaptive equipment, and compensatory techniques.

• Complete accurate and timely documentation using Net Health .

• Collaborate with physicians, nursing staff, and the interdisciplinary care team to achieve the best possible resident outcomes.

• Maintain compliance with all federal, state, and facility regulations.

• Current Pennsylvania Certified Occupational Therapy Assistant (COTA) license.

• Graduate of an accredited Occupational Therapy Assistant program.

• Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply .

• Experience with Net Health is a plus.

• Strong communication, organizational, and teamwork skills.
