Description

• Conduct thorough evaluations of patients in either inpatient or outpatient settings and devise personalized treatment plans
• Implement physical therapy interventions to enhance mobility, strength, balance, and functional capabilities
• Collaborate with a multidisciplinary team including nurses, physicians, case managers, and rehabilitation specialists to optimize patient outcomes
• Provide education to patients and their families about therapeutic objectives, discharge planning, and at-home exercise regimens
• Maintain accurate documentation of evaluations, treatments, and progress in line with facility and regulatory standards
• Engage in care team meetings and interdisciplinary conferences to ensure cohesive care delivery

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• Current state licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications in areas such as neurological or geriatric therapy are advantageous, though not mandatory

Skills

• Demonstrated experience in a similar role is highly preferred
• A deep understanding of complex medical and mobility challenges
• Excellent communication and teamwork abilities
