Job summary: The Care Coordinator/Care Manager for the "Children's Health Home program" is the key point of contact for managing services for the youth. A "Health Home" is not a physical place; it is a care team of health care and service providers working together to make sure the care and services are provided to promote better quality of life. The Care Coordinator/Care Manager is responsible for connecting the youth with healthcare, mental health providers as well as other community resources. As a liaison, the Care Coordinator/Care Manager makes referrals to in home and community services, advocates for the youth's needs, schedules appointments and transportation, collaborates with caregivers, schools, other service providers regularly to ensure all needs are met. Populations we service include youth that have behavioral health needs, experienced complex trauma, live with developmental disabilities, or are medically fragile. The caseload consists of 16-20 children under 21 years of age.
Essential job duties:
- Completes comprehensive assessments objectively, that identifies individual, medical, and behavioral health needs within appropriate timeframes.
- Develops plans of care which clearly identifies goals and timeframes to put them on the road to better physical and/or mental health.
- Conducts face to face visits with the child/youth and/or family monthly to ensure needs are addressed and assess if additional resources are needed.
- Delivers care coordination/care management which meet the needs of the child, consider the child's family and community contexts, and are developmentally appropriate, strengths based and child specific.
- Facilitates collaboration through regular case review meetings and utilizes electronic medical records such as Healthelink and Psyckes. Promotes interdisciplinary collaboration among all providers, the child/youth, family, caregivers, and local supports.
- Ensures that services recognize that the family is the primary support system for the child and participates as a full partner in all stages of decision making and treatment planning process through OLV's trauma informed practices.
- Demonstrates the ability to utilize an electronic health record system to maintain comprehensive and detailed client records which include but are not limited to the plan of care, case notes, and assessments.
- Ensures that OLV Human Services policies, procedures, and all applicable governmental laws, regulations, and guidelines are followed.
- Performs all other duties as assigned.

Competencies template: Professionals education & experience:
- Bachelors with two (2) years of relevant experience or,
- Masters with one (1) year of relevant experience or,
- Licensed as a Registered Nurse (RN) with two years of relevant experience
Relevant work experience is defined as providing direct services for children in one or more of the following primary diagnoses: mental illness, developmental disability, alcoholism, chemical dependency, or substance abuse. Required license/registration/certification:
- CANS-NY and HCBS assessment tool
- The Care Manager will be required to become CANS-NY and HCBS certified and must achieve at least a score of 70% or higher on exam upon hire. Recertification will be required on an annual basis.
- NYS Driver's License
- Candidates must have a valid New York State Driver's License and reliable transportation as they will be required to utilize a personal vehicle for company business

Physical requirements:
- The overall nature of the position is sedentary requiring little physical effort with occasional light physical exertion required.
