Physical Therapist

Position: Physical Therapist Salary Range: $38-41 per hour Hours: 40 hours per week, Monday-Friday with weekend rotation Facility: 74-bed skilled nursing facility About the Facility Our facility is a 74-bed skilled nursing facility that offers post-hospital, short-term rehabilitation, as well as long-term and respite care services. We are dedicated to providing high‑quality care to our residents and helping them achieve their maximum level of independence. Job Description We are seeking a skilled and compassionate Physical Therapist to join our team. The ideal candidate will have experience working in a skilled nursing facility and a passion for helping patients achieve their rehabilitation goals. Responsibilities Evaluate patients' physical abilities and develop individualized treatment plans Implement treatment plans to improve mobility, strength, balance, and coordination Utilize therapeutic exercises, manual therapy, and other modalities to help patients regain function and reduce pain Monitor and document patients' progress and adjust treatment plans as needed Communicate with patients, their families, and other healthcare professionals to coordinate care and ensure the best outcomes Provide education to patients and their families on how to continue their rehabilitation at home Supervise and delegate tasks to physical therapy assistants and support staff Qualifications Valid state Physical Therapist license Minimum of 1 year of experience in a skilled nursing facility Strong knowledge of rehabilitation techniques and modalities Excellent communication and interpersonal skills Ability to work independently and as part of a team Compassionate and patient‑focused approach to care #J-18808-Ljbffr Trinity Healthcare Talent Services
