Requirements

• Active California RN License (No Temporary License)
• BSN Required
• BLS
• Minimum 3 years of recent Case Management experience
• Epic experience
• First-time travelers accepted with required experience

Preferred Qualifications

• Teaching Hospital experience
• Trauma Level I experience
• Acute Hospital Case Management experience
• Experience with Med/Surg patients
• Knowledge of InterQual Criteria
• CMS and Joint Commission standards

Responsibilities

• Coordinate care for patients throughout hospitalization.
• Develop and implement individualized discharge plans.
• Collaborate with multidisciplinary teams to ensure continuity of care.
• Support liver failure patients through the Emergency Department, Observation, and Inpatient units.
• Work closely with Community Health Workers (CHWs) to coordinate care and discharge planning.
• Ensure compliance with CMS, Joint Commission, and hospital case management standards.
