Social Worker responsibilities include: Planning, coordinating, and implementing behavior intervention plans to help clients, their families, and care team thrive in their environment Interviewing clients and clients' stakeholders and assessing their current condition, needs, strengths and weaknesses Providing services that address the various needs of the clients and families through setting tailored measurable goals Working with children and adults with disabilities in the home, residential, and community settings.
