Opportunity Multnomah County Health Department is seeking a visionary and dynamic leader in Integrated Clinical Services. The role focuses on comprehensive primary medical care for medically underserved communities, addressing health equity, prevention, and chronic disease management. Responsibilities Conduct comprehensive health assessments, order and interpret diagnostic tests, and develop treatment plans for acute and chronic conditions. Utilize panel‑based information technology tools to prioritize proactive care, including preventive health screenings and chronic disease management. Follow up on utilization issues such as emergency department use and hospital admissions to improve patient outcomes. Maintain training on new or updated systems to support patient health maintenance and enhance quality of care. Identify needs for specialty and rehabilitative services and make appropriate referrals, documenting medical necessity in the EHR. Assess and counsel patients on preventive care and schedule high‑risk referrals when necessary. Provide clinical consultation to nurse practitioners, physician assistants, nurses, and support staff, prioritizing and directing the care team. Coordinate care with behavioral health providers to deliver integrated services. Provide coverage for clinical colleagues during absences and collaborate on clinic, department, and county‑wide initiatives. Complete patient documentation, manage electronic inbox, and tag records for follow‑up and collaboration. Enter encounter and event codes for accurate billing of internal and external medical services. Qualifications Graduation from an accredited medical school and one year residency in a primary‑care setting. Board certified or board eligible in Family Medicine or Internal Medicine. Valid Oregon medical license and credentialing approval. CPR certification. Experience working with underserved populations, including knowledge of African‑American community culture.
