Registered Nurse Case Manager

Acute Care Case ManagerThis is a 13-week contract position.Day shift, 5x8 hours (08:00 - 16:30). Weekend requirements based on departmental needs (every other weekend).The Acute Care Case Manager will be responsible for providing effective care coordination, discharge planning, and utilization review for hospitalized patients.
