12hrs Shifts. +Rotating Weekends

Responsibilities:

• Provide direct patient care to residents in our assisted living and skilled nursing facilities, including administration of medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Communicate effectively with residents, families, and healthcare professionals to ensure continuity of care
• Participate in resident assessments, care planning, and quality improvement initiatives
• Maintain accurate and timely documentation of resident care and services
• Collaborate with the nursing team to ensure seamless transitions and communication

Requirements:

• Current Registered Nurse (RN) license in the State of Missouri
• Experience in Long Term Care, senior living or related setting preferred.
• Strong communication and interpersonal skills
• Compassionate and empathetic approach to patient care
