Kaiser Permanente is seeking an RN Case Manager Liaison Nurse for inpatient discharge planning at St. Peter Hospital in Olympia. This role focuses on planning discharges and follow-up care for patients, requiring effective collaboration with care teams to improve patient outcomes. The ideal candidate should have a minimum of three years of clinical experience and at least two years in case management. A bachelor's degree and Washington RN license are required.
