Practice Model

• Provide comprehensive patient assessments and individualized care plans
• Manage and coordinate care for post-acute patients in collaboration with interdisciplinary teams
• Utilize population health data to guide quality care and outcomes
• Participate in a structured 3-month onboarding program with dedicated clinical mentorship

Qualifications

• Active New York medical license and DEA
• Experience in post-acute, acute care, ED, or clinic settings
