Practice Model
- Provide comprehensive patient assessments and individualized care plans
- Manage and coordinate care for post-acute patients in collaboration with interdisciplinary teams
- Utilize population health data to guide quality care and outcomes
- Participate in a structured 3-month onboarding program with dedicated clinical mentorship
Qualifications
- Active New York medical license and DEA
- Experience in post-acute, acute care, ED, or clinic settings
