Program Coordinator

Serve as the primary Hub spokesperson and sustain engagement with providers and community partners Convene Hub members to assess local needs, set priorities, and develop a tailored Hub strategy Manage localized program implementation, budgets, delivery-partner coordination, and rollout of statewide initiatives at the Hub level Monitor Hub progress and performance and escal… Oversee RHTP Navigator utilization, workflows, closed-loop referrals, outreach, documentation, and alignment with Hub priorities Facilitate Leadership Board governance, including agendas, meetings, decision documentation, and action-item follow-up Coordinate onboarding, participation expectations, and operational alignment across hospitals, FQHCs/RHCs, behavioral health providers, EMS, pharmacies, public health agencies, schools, and community organizations Develop and implement referral pathways, communication processes, escalation protocols, and operational workflows Track and report operational, implementation, referral, and performance data using dashboards, CIE systems, and approved reporting tools Identify operational barriers, workflow inefficiencies, and service gaps and coordinate improvement activities Maintain a visible local presence through stakeholder meetings, community events, and outreach Support implementation readiness, partner onboarding, workflow testing, operational coordination, and launch preparation Perform other duties as assigned Requirements Associate's Degree in Community Health, Public Health, Social Services, Behavioral Health, Human Services, or a related field — Required 1–3 years of experience in healthcare operations, community health care coordination, public health, quality improvement, or related field, in lieu of an Associate's Degree Previous experience in community health work, care coordination, case management, patient navigation, Medicaid care management, behavioral health support, public health outreach, social services, or related settings — Required Experience building and sustaining partnerships across healthcare providers, EMS, public health agencies, behavioral health organizations, community-based organizations, schools, and social service providers — Required Familiarity with care coordination workflows, social determinants of health, community resource navigation, and/or closed-loop referral systems — Required Valid driver's license required upon hire Ability to travel between various system facilities and off-site locations as needed — Required Ability to coordinate multi-stakeholder initiatives involving healthcare providers, public agencies, and community organizations Comfort using digital platforms, dashboards, electronic referral systems, and other technology tools Strong analytical and organizational skills, including experience using data and reporting tools Strong verbal and written communication, facilitation, relationship-building, professionalism, diplomacy, and public speaking skills Core Competencies Demonstrates expertise in community health coordination, partnership building, and operational management within healthcare settings. Proficient in utilizing data reporting tools and facilitating multi-stakeholder initiatives to enhance service delivery and community engagement.
