You will conduct assessments, coordinate IFSPs, and support eligibility and referrals while maintaining compliant records in a collaborative team environment. Based in Valier or nearby, you will partner with families, providers, and stakeholders to enable access to services, ensure timely delivery, and advocate for families' rights and choices, contributing to #J-18808-Ljbffr Benchmark Human Services
