Licensed Practical Nurse (LPN) - Long Term Care

Schedule: Mid shift, 5x8-hour (15:00 - 00:00)
Assignment Details: Provide care and service to a minimum of 20 and a maximum of 40 residents.
Requirements
- Current LPN License
- Experience in long-term care preferred
- Basic life support (BLS) certification
Responsibilities
- Provide direct patient care, including assistance with activities of daily living.
- Monitor residents' well-being and report changes to the supervising RN or physician.
- Administer medications as prescribed and document accurately.
- Provide wound care and assist in specimen collection for lab testing.
- Supervise and direct CNAs in daily resident care tasks.
- Participate in care planning and interdisciplinary team meetings.
