Physical Therapist (PT) Responsibilities

• Evaluate patients and complete comprehensive physical therapy assessments
• Develop individualized treatment plans based on physician referrals, patient goals, and clinical needs
• Provide evidence-based physical therapy interventions to improve mobility, strength, balance, endurance, and functional independence
• Monitor patient progress and modify treatment programs as appropriate
• Supervise Physical Therapist Assistants (PTAs) and rehabilitation support staff
• Collaborate with physicians, nurses, social services, and interdisciplinary healthcare teams
• Participate in care planning meetings, discharge planning, and family conferences
• Educate patients, families, and caregivers regarding treatment plans, exercise programs, and safety strategies
• Maintain accurate, timely documentation in accordance with regulatory and facility standards
• Recommend and assist with adaptive equipment and durable medical equipment needs
• Ensure compliance with Medicare, state, federal, and facility guidelines

Qualifications

• Current California Physical Therapist (PT) License in good standing
• Graduate of an accredited Physical Therapy program
• Bachelor's Degree required; Master's or Doctorate in Physical Therapy preferred
• Experience in Skilled Nursing Facility (SNF), Post-Acute Care, Rehabilitation, Long-Term Care, or Acute Care settings preferred
• Strong patient assessment and treatment planning skills
• Excellent communication and documentation abilities
• Knowledge of EMR systems such as Casamba or PointClickCare preferred
• Ability to work effectively in a collaborative healthcare environment
• New graduates with strong clinical experience are encouraged to apply
