Location Chestertown, MD, 21620 (Kent & Queen Anne's Counties, Maryland) Duration 6 months with possible contract extension Work Schedule Shift: 8:00 AM – 4:30 PM Weekly Hours Guarantee: 40 hours Mileage Reimbursement Provided between patient visits Job Summary We are seeking an experienced Registered Nurse (RN) to provide comprehensive home health care services across Kent and Queen Anne's counties. The RN will manage patient care episodes, complete OASIS documentation, and ensure high-quality coordinated care for home health patients. Key Responsibilities Clinical Care & Case Management Provide skilled nursing care in accordance with physician orders and patient care plans Manage a caseload of home health patients, ensuring continuity and quality of care Perform comprehensive patient assessments and monitor changes in condition OASIS & Visit Types Complete and submit OASIS documentation for admissions, discharges, and resumption of care Conduct routine visits (RV) and ensure accurate clinical documentation Care Coordination Collaborate with physicians, therapists, and interdisciplinary team members Communicate patient progress, concerns, and care plan updates effectively Coordinate services to ensure optimal patient outcomes Documentation & Compliance Maintain accurate and timely documentation in compliance with regulatory and payer requirements Adhere to infection control protocols and agency policies Ensure all clinical activities meet state and federal home health standards Coverage Area: Kent County, MD Queen Anne's County, MD Qualifications Active Registered Nurse (RN) license Home health experience (mandatory) Strong knowledge of OASIS documentation Ability to work independently and manage multiple patients Preferred Qualifications Experience with Homecare Homebase (HCB) Strong case management and organizational skills Excellent communication and patient education abilities #J-18808-Ljbffr The Judge Group
