Responsibilities
- Plan, organize and direct home care services to meet patients' needs in their homes
- Manage a patient caseload with efficiency and effectiveness
- Provide patient care independently while collaborating with core disciplines to meet the patient's plan of care
- Serve as a resource to patients, families and other care team members
- Document patient progress, update care plans, and coordinate with physicians and other providers as needed

Requirements
- Active California RN license
- Basic Life Support (BLS) certification
- Experience in case management or home health care preferred
- Ability to manage an independent caseload and coordinate with interdisciplinary teams
- Strong communication and documentation skills; capacity to work in a home-based care environment
