Home Health Registered Nurse | Newton Center, Massachusetts

Schedule:

• Weekend rotations required: once every 6 weeks

Qualifications:

• Active RN license
• At least 1 year of certified home care experience
• Minimum 1 year of case management experience
• Strong clinical assessment and critical thinking skills
• Ability and willingness to travel as required for case assignments
• Excellent communication and documentation skills
• Proficient with electronic medical records and interdisciplinary coordination

Responsibilities:

• Perform comprehensive patient assessments, accurately identifying needs and changes in condition
• Develop, implement, and adjust individualized care plans according to physician orders and best practices
• Provide education, support, and ongoing communication to patients and families to ensure understanding and engagement in care plans
• Carry out admission processes and coordinate ongoing case management in line with current standards and organizational policies
• Maintain timely, thorough documentation through electronic health records
• Collaborate closely with the interdisciplinary team to ensure optimal patient outcomes
• Uphold organizational philosophy, fostering patient safety, independence, and well-being
