Licensed Practical Nurse (LPN), PRN

Role: Per Diem Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in Home Health, providing quality patient care in patients' homes. Deliver skilled nursing services within the scope of practice defined by state laws, following the plan of care, and collaborating with the healthcare team. Educate patients' families on care procedures and maintaining a healthy environment. Observe, document, and report patient responses and condition changes to supervising nurses or physicians. Adapt to diverse home environments, ensuring equitable care regardless of age, race, creed, or background. Qualifications & Skills Current unrestricted LPN or LVN license in the state of practice. Valid driver’s license, vehicle insurance, and reliable transportation or access to public transit. Preferred: CPR certification, home care experience, ability to work independently, strong communication and organizational skills, and flexibility. Key Duties Perform patient visits and non-visit activities, with compensation based on productivity points and hourly rates.
