Hospice RN: Care Coordinator & Bereavement Support

VITAS Healthcare

VITAS Healthcare is seeking an RN to coordinate and deliver comprehensive hospice care to patients and families. The role involves conducting comprehensive assessments, developing the care plan with the interdisciplinary group, and ensuring palliation of end-of-life symptoms in alignment with the plan of care.
