ADA: Full time position. Ability to sit for long periods of time and/or stand periodically during work day and lift up to 30 lbs., when necessary. Computer data entry a portion of job duty.

OSHA: Must adhere to universal precautions, to include blood borne pathogen protection, at all times.

Position Summary: The value-based clinician is a unique position that will collaborate closely with care management, pharmacy, and Regional Medical Officers to identify unique patients with complex chronic conditions. Looking for a high energy and eager clinician who is excited about a unique model of care that combines traditional clinic based work with clinic based virtual care and home visits. The main focus is with patients across our organization with uncontrolled diabetes. This clinician will split time in a traditional clinic setting and in a virtual or outreach type setting. They will often times perform proactive outreach to patients with uncontrolled diabetes followed by other clinicians who have been identified by care management, pharmacy, and RMOs as needing further medical management. Clinician will appropriately adjust medications, communicate changes with patient and primary care clinician, and monitor patient at a regular cadence to re-adjust medications as needed. Clinician will also be involved in completing prior authorizations, doc to docs, and other tasks needed to ensure patient is able to receive specialty medications that they may require.   The clinician will model the organizational values at all times.

Job Description:

Role and Responsibilities Clinician Care Management 

• With your team, independently perform outreach to patients with complex chronic care conditions needing attention. Often times this will be virtually, but at times will be in person or through visiting the patient’s home to deliver care.  
• Travel to different communities within our wide service area to reach patients who may need care at home. 
• High energy, entrepreneurial, and interested in having a strong clinical presence, make impact at scale, and be part of a new way of caring for our patients. 
• Participate in telehealth, refilling medications, and prescribing new medications to patients with care gaps (for patients followed at the center, new patients, and patients followed by clinicians at other centers in our service area) 
  

Role and Responsibilities Clinician 

• Provide preventive care, health screenings, physical exams, laboratory testing, and immunizations in accordance with appropriate standards of care for adult and pediatric patients. 
• Perform and interpret point-of-care and diagnostic laboratory tests, and communicate results to patients. 
• Conduct comprehensive patient assessments, including medical histories, diagnostic testing, and treatment planning for acute and chronic conditions. 
• Provide reproductive health services, including contraception counseling, STI testing and treatment, pregnancy testing, and basic gynecologic care. 
• Offer education and counseling on family planning options, contraceptive methods, and preconception health. 
• Develop and implement patient care plans based on evidence-based practices and patient goals. 
• Provide accurate, evidence-based education and counseling on disease prevention, medication adherence, and self-management of chronic conditions. 
• Proactively deliver care in dynamic environments, including patient homes, encampments, and community settings in collaboration with local partners
• Maintain accurate, timely, and confidential documentation in electronic health records. 
• Consult with the medical specialists as appropriate to ensure quality and safety of care. 
• Ensure compliance with all local, state, and federal regulations related to patient care, safety, and medication management. 
• Consistently closes encounters within 24 hours to generate a visit summary for patient flow and continuity of care. 
• Updates Problems, Medications, and Vaccine lists appropriately. 
• Completes ROS, anticipatory guidance, and preventive and screening components. 
• Adheres to accepted evidence-based standards of care, as applicable, and to organization developed protocols. 
• Encounters are signed at the end of the day. 
• Responds professionally to change (i.e., schedule, work assignments). 
• Attends continuing education events and shares summaries with peers. 
• Appropriately uses auxiliary departments (HBN, Care Coordination). 
• Is flexible in meeting center needs outside regular working hours (i.e., health fair, community lectures, etc.). 
• Responds appropriately to adversity or stress. 
• Renews license, privileging, credentialing, and DEA number (if applicable) in a timely fashion. 
• Addresses in basket items in a timely manner (within 24-48 hrs) 

Qualifications and Education Requirements  

Minimum Requirements (experience, training, and education): To effectively fulfill this position, candidate must meet the following requirements: 

• Current and active State of Florida, Department of Health license (APRN).
