Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 schedule.

The Outpatient Physical Therapist is responsible for evaluating all assigned patients, developing, implementing, and modifying treatment plans in accordance with patients' needs, goals, and physician direction. The therapist will also delegate and direct all discipline-specific therapy services for the assigned patients, ensuring the design of functional treatment programs to enable effective return to community activities. The role includes educating patients, families, students, coworkers, and oneself, while upholding the organization's safety and compliance standards.

• Complete a comprehensive evaluation of assigned patients.
• Develop appropriate treatment plans and goals focused on patient progress towards discharge.
• Document medical records accurately and timely, ensuring compliance with guidelines.
• Participate in student programs, providing a positive learning environment.
• Create an individualized professional development plan with clinical management.
• Adhere to discipline-specific State Practice Acts and maintain professional licensure.
• Engage in performance improvement initiatives to enhance service quality.

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and valid licensure for the state in which you will work.
