You will deliver on our commitment of excellence to our patients and help us grow through participation in continuous improvement and innovative population health activities. This position has an emphasis on customer/patient care excellence, privacy, compliance, and serving in a dynamic health setting. The direct patient care responsibilities include:Evaluating and treating patients in accordance with standards of care.Following standards of medical care and quality for patients, and monitoring care using available data and chart reviewsActing as an active participant and key source of medical expertise with the care team through daily huddlesSetting a tone of cooperation in practice by displaying a professional and approachable demeanor.Completing all medical record documentation in a timely manner working with a quality- based coder to optimize coding specificityFollowing policy and protocol defined by Clinical Leadership and BCBSAZ.Meeting with other departments to discuss quality of care, outcome data, policy, procedure, and records issuesParticipating in potential growth opportunities for new or existing servicesAssuring personal compliance with licensing, certification, and accrediting bodiesBuilding relationships with other healthcare providers, social/community organizations, and marketing partnersPerforming all other duties as assignedWorking hours may include extended hoursThis position requires 100% on-site work at the care center location and may also involve movement between clinics as neededYou Have:Current and unrestricted Nurse Practitioner license in the state of ArizonaSuccessfully completed an accredited Nurse Practitioner program and have an active Board Certification.Minimum of two years of applicable experience preferredExcellent written and verbal communication skillsDemonstrated a high level of skill with interpersonal relationships and communications with colleagues/patientsFully engaged in the concept of “Integrated team-based care” modelA willingness and ability to learn/adapt to practice in a value-based care setting.Superior patient/customer service skillsBasic computer skillsBLS certificationActive AZ fingerprint clearance cardActive Drug Enforcement Administration (DEA) licenseOur CommitmentProsano Health Solutions does not discriminate in hiring or employment on the basis of race, ethnicity, color, religion, sex, sexual orientation, gender identity, national origin, age, disability, protected veteran status or any other protected group.Thank you for your interest in Prosano Health Solutions.
