Home-Based NP/PA: Annual Wellness & Chronic Care

AbsoluteCare seeks a Quality Provider to deliver in-home annual wellness visits across Ohio, focusing on risk adjustment and detailed clinical documentation. This community-based role involves travel to member homes and may include work in the center's intermediate care area by mutual agreement.
