Join as an IRIS Consultant to support individuals in living independently through the IRIS (Include, Respect, I Self-Direct) program. Responsibilities Include Building trusting relationships with participants Developing IRIS Support and Services Plans Maintaining regular contact and conducting visits Assisting participants with IRIS eligibility and connecting with local agencies Documenting all interactions promptly Potentially conducting annual Long-Term Care Functional Screens Working independently with team collaboration Qualifications Bachelor's degree in social work, psychology, or related fields with at least one year of supervised experience with seniors or individuals with disabilities, or 4+ years of relevant social services experience, or current IRIS Consultant in good standing Bilingual skills are a plus Strong commitment to participant autonomy and respectful service Comfortable with technology and community resources Willing to serve diverse populations with empathy Requirements Valid driver’s license and reliable transportation Satisfactory driving record and auto insurance compliance This role offers benefits including insurance, paid time off, retirement contributions, loan forgiveness support, wellness resources, and more. #J-18808-Ljbffr MEP Health
