Hospice RN Case Manager

Overview Sentara Health is seeking to hire a qualified individual to join our team as a Direct Care Registered Nurse - Home Health and Hospice. This role provides high-quality nursing care to patients in their homes or hospice settings, ensuring comfort, dignity, and emotional support during end‑of‑life care.
