Job Title: Acute Rehabilitation Case Manager Profession: RN Specialty: Case Management Duration: 12 weeks Shift: Monday to Friday Hours per Shift: 8:00 AM - 4:30 PM Experience: Minimum of 2 years in case management or related field License: Current Registered Nurse or appropriate healthcare professional licensure Certifications: Case Management certification preferred Must-Have Skills/Experience: CM experience Strong communication skills Utilization review experience Ability to support team and family conferences for acute rehabilitation Description: This position is for an Acute Rehabilitation Case Manager with experience in adult acute hospital settings. The role requires strong communication skills, organizational abilities, and analytical capabilities to interpret information and carry out duties independently. The case manager will work with nursing, therapy, and medical staff to support patient care advocacy. Experience with common acute rehabilitation diagnoses, such as stroke and post-surgical conditions, is essential. Candidates should be able to balance patient care needs with resource stewardship. Effective oral and written communication skills are vital, and additional languages are preferred. A familiarity with basic computer programs, including Microsoft Excel, Word, and Outlook, is necessary. The role does not involve weekend hours or call requirements. Ideal candidates will demonstrate professionalism in challenging discussions with families and patients regarding healthcare realities.
