Physical Therapist (Outpatient) Full-Time

JOB DESCRIPTION Adventist Health Ukiah Valley has been deeply connected to the community we serve since 1956. We are comprised of a 50-bed acute care medical center, level IV trauma center, level II intensive care nursery services, extensive physical rehabilitation program, outpatient surgery and diagnostic center, and advanced wound care center. Job Summary Plans and conducts segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives. Serves as co-leader in an interdisciplinary approach to patient groups. Education and Work Experience Doctor of Physical Therapy (DPT) degree: Required; license obtained on or after January 2016 or foreign equivalent. Two years of Physical Therapy experience: Preferred. Licenses and Certifications Physical Therapy licensure in state of practice: Required. Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor per AH policy: Required. Facility Specific Licenses and Certifications Basic Life Support (BLS) certification from approved vendor per AH policy: Required. Essential Functions Complete evaluations to ensure quality standards and address assessment areas that require attention. Use appropriate clinical techniques and tools and establish individualized treatment plans for patients. Provide therapy interventions using appropriate treatment protocols and techniques. Establish individualized care and discharge plans and ensure plans are communicated to appropriate parties. Complete comprehensive and accurate clinical documentation within established deadlines.
