Position Highlights 60 Physicians and 50 APPs fully integrated across 30 practice sites Outpatient primary care with telephone call that averages once every 30 days Office hours are Monday–Friday. Specific hours vary by practice. The APRN/PA may function as a PCP maintaining a panel and/or via a care team model co‑managing patient panels with physician partner(s). The APRN/PA participates as part of an interdisciplinary team to provide safe, high quality, cost‑effective, and evidence‑based care. The APRN/PA demonstrates the highest levels of professionalism and upholds the missions, visions, and values of Advocate Health.
