Requirements:

• Graduate of an accredited Physical Therapy program.
• Current Physical Therapist license in the state of practice.
• Experience in skilled nursing facility (SNF) or geriatric rehabilitation preferred.

Roles & Responsibilities:

• Evaluate residents and develop individualized treatment plans to improve mobility, functional independence, and quality of life.
• Deliver skilled physical therapy services in compliance with physician orders, facility policies, and applicable regulations.
• Monitor residents' progress and adjust care plans as necessary to achieve therapy goals.
• Educate residents, families, and caregivers on home exercise programs and safety strategies to promote independence.
• Supervise and guide Physical Therapist Assistants (PTAs) and therapy aides.
• Document treatment sessions, progress notes, and billing information accurately and in a timely manner.
