Part-Time Health Services Coordinator (LVN/LPN) Pay: $32.00 - $35.00/hr. Schedule: Part Time, Saturday - Monday, 8:30 am - 5:00 pm. Job Summary The Health Services Coordinator serves on the health services team supporting the overall operations of the department through a variety of administrative and clinical functions as directed by the Director of Health Services. The coordinator preserves dignity and promotes independence for each resident while providing care and services according to each individual service plan and in accordance with company policy and procedure. Duties & Responsibilities Conduct and coordinate assessments/evaluations of potential residents and make recommendations for admission in accordance with current rules, regulations, and policies. Draft initial individualized Service Plans and update as needed. Review service plans with responsible parties and the Executive Director as requested. Ensure continuity of the assisted living residents’ total care regimen. Provide training and education on medication delivery, treatment protocols, safety procedures, accurate record keeping, and state requirements. Perform all assigned duties accurately and timely including required documentation. Serve as a medication technician, if needed. Provide coverage of job duties during absences. Check vital signs as directed and monitor health status. Perform basic nursing functions such as treatments, medication delivery, and emergency management. Maintain adequate inventory of resident care supplies and minimize waste. Coordinate prescription orders with doctors and manage pharmacy delivery, including oversight of central storage and Medication Administration Records. Aid in community marketing through positive interactions and liaison with families and outside providers. Maintain a safe and secure environment for staff, residents, and guests. Understand and comply with all applicable regulations and company policies. Promote teamwork and open communication in accordance with company principles and core values. Perform other job duties or special projects as assigned. Education Requirements RN, LVN, or LPN license that is active and in good standing; adherence to all requirements to maintain license including CEU completion and timely renewal. Experience Requirements 2+ years of related work experience in a similar healthcare environment. Required Competencies, Licenses, and Certifications Complete required background clearances, health screening and provide negative TB test results within 7 days of employment. Valid state driver's license and reliable transportation. Competent and current technical and computer skills, including familiarity with Microsoft Office Suite. Excellent communication skills in speaking, writing, and reading English. Ability to make sound independent decisions during circumstances warranting calmness in stressful situations. Ability to deal tactfully and professionally with personnel, residents, families, and guests. Strong organizational, problem‑solving, and time‑management skills. Maintain a neat and organized work environment to promote safe coordination of resident care. Physical Demands & Work Environment Work a flexible schedule, including weekends and holidays. Mobile and able to perform physical requirements: walking, bending, kneeling, squatting, pulling, reaching overhead, repetitive motion. Move intermittently throughout the day and between locations.
