You'll provide skilled, one-on-one care within a dedicated territory while partnering with an experienced interdisciplinary team.

A Day in the Role

As a Home Health Registered Nurse, you will:

• Complete patient assessments and manage an assigned caseload
• Develop and update individualized Plans of Care
• Provide skilled nursing treatments and medication education
• Monitor and communicate changes in patient condition
• Coordinate care with physicians, therapists, and other providers
• Help reduce avoidable hospitalizations
• Maintain timely and accurate documentation

You're a Great Fit If You Have

• Current, unrestricted RN license in Massachusetts
• One year of experience in a clinical setting
• Valid driver's license, dependable vehicle, and current insurance
• Ability to travel throughout your assigned territory
• Be able to work independently while being a part of a collaborative team
• Sit, stand, bend, lift, and move intermittently and be able to lift 50100 lbs.
