Get to Know Your Team: Portneuf Medical Center (PMC) is a nationally recognized healthcare leader with 205 beds, serving as the region's Level II Trauma Center and tertiary referral hospital, and is home to the Portneuf Heart and Vascular Institute, Portneuf Cancer Center, Portneuf Medical Group, and Portneuf Air Rescue. PMC has a Level III neonatal intensive care unit, an adult intensive care unit, nationally recognized orthopedic services, a full suite of cardiac care, Center of Excellence recognized surgeons, imaging, primary and specialty care. Responsibilities The clinical pharmacist specialist is responsible for the provision of safe, effective, and prompt medication therapy. Provides patient-centered care and integrated pharmacy services in a decentralized practice setting with physicians, nurses, and other hospital personnel. Participates in all necessary aspects of the medication-use system while providing comprehensive pharmaceutical care to the patients in the assigned areas. Provides support of centralized and decentralized medication-use systems. Leads assigned patient service lines, clinical areas, and related therapeutic programs. Works as an active member of the multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas to provide robust individualized patient-centered care. Participates in daily multidisciplinary and bedside rounds when scheduled. Facilitates pharmaceutical care services and medication utilization systems within assigned services and care areas to assure drug utilization activities are aligned with patient care needs, evidence-based best practices, and regulatory standards. Educates and directs pharmacy personnel by actively participating in staff meetings and attending staff development in pharmaceutical care to identify learning and performance needs. Makes appropriate evidence-based, patient-centered medication recommendations. Must serve as preceptor for pharmacy students and/or residents and supervise and assist pharmacy technicians and other support staff effectively. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. Participates in the management of medical emergencies as appropriate, including by not limited to Code Blue response. Provides clinical consultation and clarification to practitioners. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed. Qualifications Job Requirements B.S. or Pharm D. from an ACPE-accredited School of Pharmacy. Clinical pharmacy experience in a hospital setting. Completion of an accredited PGY-1 Pharmacy Residency or fellowship AND Completion of an accredited PGY-2 Specialty Pharmacy Residency or fellowship (in an appropriate specialty field). OR Completion of an accredited PGY-1 Pharmacy residency or fellowship AND 3+ years’ clinical pharmacy experience (if no PGY-2 is completed). OR 5+ years of clinical pharmacy experience with no history of residency or fellowship. Board of Pharmacy Specialties (BPS) certification (or attained within 1 year). Active Pharmacist license in good standing with the appropriate State Board of Pharmacy. Other applicable state requirements – i.e., sterile compounding, etc. BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care. ALS/ACLS certification. Board Certification. Preferred Job Requirements Doctor of Pharmacy from an ACPE-accredited college of pharmacy. Experience working in Microsoft Office applications and Electronic Medical Record Systems. #J-18808-Ljbffr
