PT - Physical Therapist

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with patient needs and goals as directed by physicians. Responsibilities include the delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring the design of functional treatment programs that promote effective return to community activities. Responsibilities Complete a comprehensive evaluation of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals. Document care in a clear, concise, accurate, and timely manner as per clinic policies, ensuring compliance with state, federal and intermediary guidelines. Participate in the student program as assigned, providing direction and supervision while creating a positive learning environment. Actively create and pursue a professional development plan, engaging in continuing education and professional activities. Adhere to discipline‑specific State Practice Act and maintain professional licensure in good standing. Understand and participate in organizational performance improvement initiatives to enhance quality of services provided. Required Experience / Certifications / Licensure Degree in Physical Therapy from an accredited school. 1+ year of outpatient clinical experience is required. Current CPR certification. Active and applicable state licensure for the practice of physical therapy. Schedule This position requires a full‑time commitment with a 5x8.0 shift structure.
