Per Diem Physical Therapist – Mobile Division

Longevity Rehab Center

Longevity Rehab Center is seeking a motivated and reliable Per Diem Physical Therapist to join our Mobile Therapy Division . This role offers flexibility and independence, providing one-on-one outpatient physical therapy in patients’ homes and community settings.

Responsibilities:

Deliver high-quality, evidence-based physical therapy to adult and senior patients in their homes.
Evaluate, develop, and implement individualized treatment plans.
Maintain accurate and timely documentation in accordance with Medicare and company standards.
Communicate effectively with patients, caregivers, and the interdisciplinary team.

Requirements:

Licensed Physical Therapist (PT) in the state of Florida.
Reliable transportation and valid driver’s license.
Strong interpersonal and time-management skills.
Prior home health or outpatient experience preferred but not required.

Schedule & Compensation:

Per diem / flexible schedule — you choose your availability.
Competitive per-visit pay.
