Registered Nurse Case Manager

Please Note: Applicants who do not meet ALL of the qualifications listed below should not apply for this position.Required Qualifications:Must be at least 21 years of age at the time of hire.Must be willing to travel from Denver to Colorado SpringsBachelor's degree in Nursing (BSN preferred if applicable).Current Registered Nurse (RN) license in the State of Colorado with an active DORA license.Minimum of 2 years of nursing experience.Experience working with individuals with intellectual and developmental disabilities (IDD) preferred.Valid Colorado Driver's License with at least 5 years of driving experience, or ability to obtain a Colorado Driver's License within 30 days of hire.Strong decision-making, communication, organizational, and time management skills.Ability to work independently while collaborating effectively with interdisciplinary teams, families, and healthcare providers.Knowledge of applicable regulations, including Medicaid, HCPF, CDPHE, and CDHS Child Care Rules.Ability to develop and deliver staff training, including Qualified Medication Administration Personnel (QMAP) training.Proficiency with electronic medical records (EMR) and medical documentation.Ability to maintain confidentiality and exercise sound professional judgment.Must be flexible with scheduling to meet program and organizational needs.Job Summary:This position is responsible for overseeing, coordinating care and providing training to person served and support staff for those in our residential programs. While ensuring training, care plans and medications are updated to date and non-medical staff are proficient providing needed medical care to those served in our School/Day Treatment, and Adult Day Program. Also offered are 10 paid holidays, Paid Time Off, Wellness Time and a 403b retirement plan with RiseUp - match after one (1) year.(Benefits are subject to change based on reevaluation or benefit plan changes annually.)Benefits begin at first of the following month after completing a full 30 days of employment (i.e. start date 4 January 2025, benefits initiate 1 March 2025).Pay Range: $ 65,000 to 80,000 Annually paid bi-weekly based off experience and education.Shift: Monday-Friday and On-call shifts are required on weekends and after hoursDuties and Responsibilities:Uphold the mission, values, policies and procedures of RiseUP.Act as a liaison with community medical facilities, hospitals, psychiatrists, families, physicians, and other external entities in regard to the medical needs of those served.Oversee medical care for those in services.Coordinate care to support with person remaining as independent as possible.Monitor medications and health issues of individuals served by Roundup Fellowship and consult with Program Directors, Program Managers, Medical Coordinators, and other staff regarding specific needs as well as general medical issues.Monitor and obtain medical documentation such as annual physicals/physician orders etc. to ensure compliance with regulatory agencies and Roundup Fellowship’s policies and procedures.Oversee the administration of medication as needed.Review Medication Administration Records (MARS) for those served monitor/obtain physician orders.Assist person in services in the supported decision-making process regarding their healthcare needs.Train staff and provider on other medical topics to ensure needs of person in service are met while in services.Transcribe verbal physician orders and enter documents into Electric Record Management (EMR system.Participate in the admissions and discharge process and obtain or provide needed documentation.Supervise and provide guidance to the Medical Coordinators. Offer mentorship and training to improve skills and care to youth.Review the performance of Medical Coordinators, providing feedback and implementing improvement plans if needed.Coordinate ongoing education and training for staff to ensure they remain up-to-date on best practices and regulations.Ensure effective communication between program, family/guardians, individuals served, and physicians where required.Oversee the vision/hearing screening and immunizations for student(s) in the school.Reviews and ensures training and implementation of Policies and Procedures.Other duties as assigned.Required Knowledge, Skills, and Abilities:Good decision-making, time management and communication skills, and be responsible, mature, and flexible.Colorado Department of Education (CDE) School Nurse license or willing to obtain.Satisfactory background checks.Requires scheduling flexibility.Physical and Mental Demands:Ability to be physically present at job locations (Denver and Colorado Springs).The position to remain open until it is filled #J-18808-Ljbffr RiseUp
