Provide comprehensive, compassionate care to geriatric patients , focusing on improving quality of life, managing chronic conditions, and supporting healthy aging in a collaborative care setting .

Key Responsibilities

• Manage complex and chronic conditions in elderly patients
• Conduct routine health assessments and preventive care
• Develop personalized care plans focused on long-term wellness
• Address cognitive, functional, and mobility concerns
• Coordinate care with specialists, caregivers, and families
• Educate patients and families on disease management and lifestyle
• Maintain accurate clinical documentation

✅ Requirements

• Board Certified / Board Eligible in Geriatric Medicine (or Internal Medicine/Family Medicine with geriatric focus)
